Whats the difference between HMO's and CDHC? (Also Obamacare discussion)

Consumer-Driven Health Care(CDHC) seems to be whats best for health care. Clinics that are privately owned by doctors, and not HMO's, charge routine visits and generally have price listings for the kind of care. Doctors even give charitable free care in events of medical emergencies and to the poor in serious need of care but can't afford it. Its their responsibility, its part of the profession to ensure the life of others. The care comes out of the patients pockets, and private doctors don't take money from HMO's or Medicare/Medicaid.

So why don't we all use CDHC? Why are HMO's the primary system of health care in our country? I know they were created by The Government during the Nixon Administration, but how does the Government ween behind it? Also, Nationalized Health Care would be effing horrible. I think people will realize what a mistake it was to idolize Canada's health care system once they bitch about the wait lines, taxes, and choice.

And I don't quite understand Obamacare, it would only provide health care for the uninsured, and it wouldn't impact the current health care. This would either give people a strong disincentive to pay for private plans, or do almost nothing.
